The Role of Post-Reduction X-rays after Shoulder Dislocation

ABSTRACT

Objective: To determine whether post-reduction x-rays(后精简X光) add additional clinically important information over what is seen on pre-reduction films in patients with shoulder dislocations. Methods: Prospective study from 6/1/99 to 12/1/2000 of consecutive emergency department patients presenting with closed shoulder dislocations. Patients were excluded if both pre- and post-reduction films were not obtained. The shoulder reduction technique used was left to the discretion of the treating clinician. After attempted reduction, the clinician reported whether or not he/she was confident that the shoulder had been reduced. Post-reduction films were then obtained. An attending radiologist who noted the presence of dislocations and/or fractures reviewed radiographs. The radiologist was blinded to the post-reduction film until after reading the pre-reduction film. Descriptive statistics were reported. Results: 58 eligible patients were identified. 17 were excluded (15 pre-reduction films not obtained; 2 patients' post-reduction films missing) 41 patients were enrolled. The clinician thought the shoulder was clinically relocated in 40 patients, and was unsure in 1 patient. In each case the shoulder was adequately reduced on post-reduction films. 13/41 (32%) patients had fractures identified on the pre-reduction films vs. 18/41 (44%) who had fractures identified on the post-reduction films. Of the 5 new fractures identified by post-reduction films, 4 were Hill-Sacks or Bankhart fractures and 1 was a glenoid fracture. Conclusions: Clinicians accurately predict whether a shoulder dislocation has been adequately reduced. However, 28% of identified fractures were visualized only on the post-reduction film. Post-reduction films yield clinically important information compared to pre-reduction films alone in patients with shoulder dislocations.
